Complete a Child Support Application Form (Form 101)

	EITHER Lodge the Form with the Child Support Agency for collection OR Complete a Child Support Agreement C2.

	NOTE:  If you are a Centrelink recipient, you must have this Agreement approved by Centrelink first, and collected by the CSA
